Role Description Under the direction of the Division of Mental Health Adult Partial Care Program Director, the APC Case Manager (CM) provides services geared toward assisting individuals to work toward their valued social role(s). The CM is responsible to provide services consistent with the agency’s mission and wellness / recovery principles. The CM should be able to report the status of their caseload of consumers at any time, including crises and psychiatric concerns, to the Program Director. The CM should also be a team player, and be able to accept feedback and, at times, constructive criticism as a part of the supervision process. Documentation timeliness is an integral part of this position. Responsibilities As a Mental Health Case Manager in our mental health day program, you’ll help clients build skills, achieve personal goals, and move forward on their recovery journey. Your responsibilities will include: Facilitate therapeutic and educational groups. Provide individualized case management services. Assist clients with developing and achieving recovery goals. Support individualized recovery planning and service coordination. Connect clients with community resources and support services. Encourage independence, personal growth, and community integration. Provide occasional transportation for clients as needed. Collaborate with an experienced, multidisciplinary team to support client success. Maintain appropriate documentation and service records. Qualifications Must have, at a minimum: i. A bachelor's degree in human services discipline, rehabilitation, or social work; ii. A bachelor's or an associate degree in psychosocial rehabilitation or mental health services; iii. A Bachelor of Science degree in nursing; iv. An associate degree and two years' experience in providing human services ; or v. A relevant professional credential, such as certified psychiatric rehabilitation practitioner, certified rehabilitation counselor, certified alcohol and drug counselor, certified alcohol and drug counselor-intern, or community mental health associate; #J-18808-Ljbffr Helping Hand Behavioral Health
