Job Summary/Objective:
The Registered Dietitian Nutritionist spends approximately half of their time providing Diabetes Self-Management Education and Support (DSMES), including Medical Nutrition Therapy and individualized diabetes education. The remainder of their time is spent providing clinical Medical Nutrition Therapy for patients with other nutrition-related conditions, interdisciplinary care coordination, and supporting organizational nutrition initiatives. Performs all defined services and other related duties in accordance with the mission of Peoples Community Health Clinic.

Diabetes Care and Education (CDCES)
- Provide individual and group Diabetes Self-Management Education and Support (DSMES) consistent with current evidence-based standards.
- Develop and deliver individual and group diabetes education that addresses healthy eating, carbohydrate counting, medication adherence, physical activity, behavior change, glucose monitoring, continuous glucose monitoring (CGM), insulin administration, insulin pump therapy, and other diabetes technologies, as appropriate.
- Interpret glucose data, laboratory values, and nutrition-related clinical indicators to develop and monitor individualized diabetes management plans.
- Collaborate with the interdisciplinary care team to optimize diabetes outcomes and support diabetes-related quality improvement initiatives.
Clinical Nutrition Assessment and Medical Nutrition Therapy
- Conduct comprehensive nutrition assessments utilizing medical, nutritional, behavioral, social, cultural, and environmental information.
- Develop, implement, monitor, and modify individualized nutrition care plans using the Nutrition Care Process and evidence-based practice guidelines.
- Provide Medical Nutrition Therapy for patients with nutrition-related conditions across the lifespan, including maternal, pediatric, adult, geriatric, and oral health-related nutrition needs.
- Document nutrition assessments, interventions, patient education, and follow-up care within the electronic health record.
Interdisciplinary Care Coordination
- Collaborate with providers and members of the interdisciplinary care team to develop coordinated patient care plans, provide nutrition consultation, and support evidence-based nutrition interventions.
- Collaborate with the Community Nutrition Specialist to integrate Food Is Medicine, Produce Prescription, and other nutrition access programs into individualized patient care plans, coordinating referrals as appropriate.
Patient Education
- Provide individualized nutrition counseling utilizing motivational interviewing and evidence-based behavior change strategies.
- Develop and deliver culturally responsive nutrition education for patients, families, and caregivers.
Population Health and Quality Improvement
- Participate in quality improvement initiatives, population health activities, and development of nutrition-related clinical protocols and evidence-based practice guidelines.
- Monitor and support achievement of organizational quality measures related to nutrition services, including diabetes, hypertension, obesity, preventive care, and UDS reporting.
Administrative
- Maintains current and quality written nutrition services materials, policies, and procedures.
- Supports a service-oriented atmosphere in accordance with Peoples Community Health Clinic Mission and Philosophy.
- Works to improve work processes and clinical outcomes including health disparity and quality improvement collaboratives.
- Follows policies and procedures.
- Maintains a safe working environment and practices safe working habits.
- Assists in control of Peoples Community Health Clinic resources.

- Working knowledge of MS Office Products (ex. Excel, Word, Power Point) and Windows Platform. Must be comfortable moving within an EHR computer system regularly
- Maintains strict confidentiality.
Supervisory Responsibilities:
- Provide day-to-day supervision, mentoring, and clinical guidance to the Community Nutrition Specialist.
- Support orientation, performance evaluation, workflow coordination, and professional development of the Community Nutrition Specialist.
- Ensure community nutrition programming aligns with evidence-based practice, organizational priorities, and interdisciplinary care plans.
Patient Population:
Pediatric, adolescent, adult, and geriatric patients.
Work Environment/Personal Protective Equipment:
This position operates in a clinical environment, PPE required as needed.
Physical Demands:
This position requires continuous sitting (67% - 100% of shift); occasional standing and walking (1% - 33% of shift); occasional lifting or carrying up to 20 lbs. and pushing/pulling up to 100 lbs. (1% - 33% of shift); continuous finger dexterity and use of hands/arms for repetitive movement (67% - 100% of shift); occasional reaching at, above, and below should level. (1% - 33% of shift).
Position Type/Expected Hours of Work:
This is a full-time position. Typical work hours and days are between Monday and Friday,8:00 a.m. to 5:00 p.m. and some flexibility in hours is allowed. Responsibilities occasionally may require irregular hours as deemed necessary. This may include an adjusted work schedule, long days, and early morning or late evening hours in order to meet client scheduling demands. The employee must work a minimum of 40.0 hours each week to maintain full-time status.
Travel:
Occasional driving might be expected for this position.
Education and Experience:
- Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR) required.
- Certified Diabetes Care and Education Specialist (CDCES) credential preferred at the time of hire but required to obtain within twenty-four (24) months of hire, with employer support for certification and continuing education.
- Experience providing Medical Nutrition Therapy in an outpatient or primary care setting preferred.
- Experience in a Federally Qualified Health Center (FQHC), community health center, or other underserved setting preferred.
- Experience with diabetes technology, including continuous glucose monitoring (CGM) and insulin pump education preferred.
- Experience with HRSA quality measures, population health initiatives, and addressing social drivers of health preferred.
Licenses/Certifications:
- Current licensure as a Dietitian in the State of Iowa.
- Current BLS or CPR/First Aid certification
- Current mandatory reporter training for child and dependent adult abuse certificate
