Licensed Independent Clinical Social Worker (LICSW)

Lifestyle Management, Weight Management, Cardiopulmonary Rehabilitation, and Diabetes Services

The Licensed Independent Clinical Social Worker (LICSW) is a key member of the multidisciplinary care team, providing behavioral health services and psychosocial support to patients participating in Lifestyle Management, Weight Management, Cardiopulmonary Rehabilitation, and Diabetes programs. The LICSW promotes whole-person care by addressing emotional, psychological, behavioral, and social factors that affect health outcomes, self-management, and quality of life.

Working collaboratively with physicians, advanced practice providers, nurses, registered dietitians, exercise physiologists, pharmacists, and other healthcare professionals, the LICSW assists patients in overcoming barriers to lifestyle change, chronic disease management, and treatment adherence.

Essential Responsibilities

Clinical Assessment & Intervention

• Conduct comprehensive psychosocial assessments to identify behavioral, emotional, environmental, and social determinants impacting health.
• Evaluate patients for depression, anxiety, adjustment disorders, stress, substance use concerns, and other psychosocial issues affecting treatment participation.
• Develop individualized care plans that support patient goals and improve self-management skills.
• Provide short-term counseling utilizing evidence-based interventions, including:
  • Motivational Interviewing (MI)
  • Cognitive Behavioral Therapy (CBT) techniques
  • Solution-Focused Therapy
  • Behavioral activation and coping strategies
  • Stress management and resilience training

Weight Management & Lifestyle Medicine

• Support patients pursuing weight loss and sustainable lifestyle changes.
• Address emotional eating, binge eating behaviors, body image concerns, and barriers to behavior change.
• Facilitate goal setting and behavior modification strategies to improve adherence to nutrition, exercise, and wellness plans.
• Collaborate with providers to promote long-term weight management success.

Cardiopulmonary Rehabilitation

• Provide psychosocial support to patients with cardiac and pulmonary disease participating in rehabilitation programs.
• Assist patients in adjusting to new diagnoses, chronic illness, disability, and lifestyle changes following cardiac or pulmonary events.
• Screen for depression, anxiety, and emotional distress associated with chronic cardiopulmonary conditions.
• Facilitate support groups and educational sessions focused on coping skills and self-management. Consistent with program descriptions, LICSW support is integrated into cardiac and pulmonary rehabilitation services.

Diabetes Care & Chronic Disease Management

• Assist individuals living with diabetes in managing the psychological and behavioral aspects of chronic disease.
• Address diabetes distress, treatment fatigue, self-management challenges, and health literacy barriers.
• Collaborate with diabetes educators, dietitians, pharmacists, and providers to improve treatment adherence and health outcomes.
• Connect patients with community resources and support services that promote disease management and healthy living.

Care Coordination & Resource Management

• Identify social determinants of health affecting patient outcomes, including:
  • Financial concerns
  • Food insecurity
  • Transportation barriers
  • Housing instability
  • Caregiver stress
• Facilitate referrals to behavioral health providers, community agencies, support groups, and social service organizations.
• Coordinate care transitions and ensure continuity of services across care settings.

Group Facilitation & Patient Education

• Develop and facilitate educational and support groups for patients and families.
• Lead discussions on:
  • Behavior change
  • Coping with chronic illness
  • Stress reduction
  • Weight management
  • Emotional wellness
  • Motivation and goal achievement
• Participate in multidisciplinary patient education initiatives.

Qualifications

Required

• Master's Degree in Social Work (MSW) from an accredited program.
• Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) license.
• Minimum of 3 years of clinical social work experience.
• Experience providing counseling and behavioral health interventions.
• Knowledge of chronic disease management and behavioral change principles.

Reporting Structure

Reports to the Director of Ambulatory Operations or designated clinical leader and works collaboratively with multidisciplinary teams supporting Weight Management, Diabetes Care, and Cardiopulmonary Rehabilitation programs.
