The team is committed to compassionate, personalized care for the communities it serves. Role Description The Home Health RN for Irving and surrounding areas is a full-time role based in Dallas, TX. The RN will perform in-home skilled nursing visits, including assessments, wound care, medication management, and monitoring of chronic and acute conditions. Day-to-day responsibilities include developing and updating individualized care plans, coordinating with physicians and interdisciplinary team members, and educating patients and caregivers on disease processes, treatments, and self-care. The RN will document clinical findings accurately and timely, ensure regulatory compliance, and contribute to high-quality patient outcomes. The role also involves participating in case conferences, supporting seamless transitions of care, and promoting patient safety and comfort in the home environment. Qualifications Strong clinical foundation in Nursing with the ability to perform comprehensive assessments and deliver evidence-based care. Experience in Home Care and patient care, including providing services in residential settings and collaborating with families and caregivers. Proficiency in Wound Care, including evaluation, treatment, and ongoing management of simple and complex wounds. Active, unrestricted Registered Nurse (RN) license in the state of Texas. Prior home health or hospice experience preferred; familiarity with CMS regulations and OASIS documentation is an advantage. Excellent communication, interpersonal, and critical thinking skills, with the ability to work independently and manage a mobile caseload. Valid driver's license, reliable transportation, and willingness to travel within Irving and surrounding areas.
