How We Work Our Culture & Core Beliefs Earn Trust Building Relationships Creating Joy Doing Right Improving Every Day Moving Forward Position Summary What You’ll Do Responsibilities Cares for Suvida Healthcare’s patients and focuses on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention Manages patients with the supervision of the Neighborhood Center Medical Director in a multidisciplinary value‑based care model Serves as the leader for their respective care team within the Neighborhood Center Collaborates effectively with Suvida Healthcare’s multidisciplinary care team Focuses on providing 5‑Star care and strives to provide a high level of customer service Keeps a medical history and medical records Refers the patient to specialists as needed Attends daily morning huddle with care team to discuss patients to be seen that day and works to close any potential care gaps Attends weekly huddle with multidisciplinary care team to review and manage high‑risk patients within the care team Begins to influence department’s strategy Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction Exercises considerable latitude in determining objectives and approaches to assignments What You’ll Bring Knowledge, Skills, and Abilities 2+ years minimum experience, preferably in geriatrics Active, unrestricted medical license in Texas (for Tx. Centers) or Arizona (for Az.
