Contract | 13 Weeks | Days | Onsite | 5x8-hour shifts (Mon–Thu 10am–7pm, Fri 9am–6pm); 4x10-hour shift schedule may also be considered

Key Responsibilities

• Complete comprehensive evaluations for assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Perform 60-minute evaluation and treatment sessions, designing functional programs that support effective return to community activities
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned
• Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain active professional licensure in good standing and fulfill all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in good standing (Florida)
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
• Commitment to patient and employee safety standards, including safe patient handling and infection control practices

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality initiatives
