Role Responsibilities

• Deliver comprehensive care for individuals and families at all life stages, spanning from pediatric to geriatric.
• Address chronic conditions, provide preventive care, and manage acute health issues with a patient-centered approach.
• Perform physical examinations, interpret diagnostic results, and craft personalized treatment strategies.
• Work closely with care coordinators, mental health professionals, and specialists to ensure integrated care.

• Utilize electronic medical records (EMR) to preserve accurate and current patient information.

Education Requirements

• MD or DO from an accredited medical institution.
• Completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine.
• Eligibility for (or possession of) an active Illinois medical license.
• BLS and ACLS certification preferred.
