Physical Therapist

Position Overview A licensed Physical Therapist providing personalized, one-on-one care to older adults in private homes or senior living communities. The role focuses on improving mobility, independence, and quality of life through evidence-based treatments within a flexible schedule that may include telehealth options. Key Responsibilities Conduct evaluations, develop individualized care plans, and deliver therapy sessions to support wellness and functional independence. Document patient progress accurately within the EMR system, adhering to regulatory standards. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth/virtual care to enhance access and continuity of care. Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state. Degree from an accredited PT program. Proficiency in computer use and EMR systems. Current CPR certification. Physical Requirements Ability to perform moderate physical effort, assist with patient transfers, repositioning, and therapeutic activities, and operate a vehicle for home visits.
