Transplant Coordinator RN - Heart Transplant and VAD 1.0 FTE Day

Transplant Coordinator RN – Heart Transplant and VAD Full‑time (1.0 FTE), day shift position at Providence St. Vincent Medical Center. Responsibilities Provide coordination and case management for ventricular assist devices (VAD) and cardiac transplant patients in collaboration with the program medical director and referring physicians to promote optimal patient outcomes. Responsible for clinical and technical support for VAD or cardiac transplant patients regarding evaluation, implant management, post‑follow‑up, and clinical training. Effectively use care conferencing and multidisciplinary transplant/VAD committee for patient evaluations and clinical reviews. Facilitate patient care within an outpatient clinic and share 24‑hour call. Coordinate patient care through medical procedures, ongoing education, inpatient and outpatient care delivery and follow‑up. Required Qualifications Education to meet certification, license or registration requirement. Oregon Registered Nurse License upon hire. VAD coordinator experience. Preferred Qualifications 3 years clinical experience with VAD patient care.
