Hammock Homes We're Hiring: Clinical Operations Manager The Opportunity You're ready to step out of the day-to-day firefighting and into true operational leadership. We're looking for a Clinical Operations Manager who can own everything that happens at Hammock Homes and thrive doing it. This is the role that will transform how we operate: someone who understands case management deeply, can lead a clinical team with authority, and has the operational chops to keep 10 case managers and site management staff, occupancy, vendor relationships, and team dynamics running smoothly. You'll report to leadership, have full autonomy over day-to-day operations, and be the single point of accountability for how Hammock Homes serves our clients and grows. About Hammock Homes Hammock Homes provides recuperative care and case management for individuals transitioning out of acute care settings such as treatment centers, emergency rooms, SNF’s, incarceration, and referral from community partners. Our mission is to help people heal and stabilize through coordinated, compassionate clinical oversight. We operate on-site facilities where our team delivers direct case management, coordinates discharge planning, manages occupancy, and ensures every client receives individualized care and advocacy. We're not a large operation, but we're intentional about quality—and we need someone who is too. What You'll Do Lead and Manage the Case Management Team. Supervise a team of case managers and site management staff. Conduct performance reviews, coach for growth, manage performance issues, and foster a culture of clinical excellence and collaboration. Ensure case managers are meeting documentation standards, client outcomes, and regulatory compliance. Own Census and Client Flow. Drive strategies to keep census full through proactive outreach, referral follow-up, and re-engagement campaigns. Monitor occupancy metrics, identify trends, and work with the team to develop and execute occupation plans. Manage Vendor and Operational Relationships. Own all vendor relationships (supplies, facility maintenance, contractors). Negotiate contracts, manage budgets, and execute purchases within approved thresholds. Solve day-to-day operational problems without escalation (within your authority). Monitor Metrics and Compliance. Establish and track KPIs for clinical quality, occupancy, financial performance, and team health. Flag issues early, report monthly to leadership, and recommend data-driven improvements. Handle Client and Staff Issues. Triage client complaints, manage difficult situations, coach staff through conflicts, and elevate only when needed. Make judgement calls autonomously on day-to-day operational and clinical challenges. Build and Improve Systems. Document processes, create playbooks, and implement systems to reduce tribal knowledge and improve consistency. Make it so the business doesn't depend on heroic efforts. What We're Looking For Required Clinical background (RN, LVN, LCSW, LMFT, or equivalent case management experience ). Strong preference for someone with a license (active or past). 5+ years of experience supervising or leading case managers or clinical staff. (REQUIRED) Experience with case management, discharge planning, and client advocacy in healthcare or social services. Proven ability to manage day-to-day operations: vendor relationships, budgets, hiring/firing recommendations, problem-solving. Comfortable with autonomy. You don't need permission to solve problems—you think critically, make decisions, and elevate only when necessary. Preferred Experience in recuperative care, behavioral health, addiction treatment, residential, skilled nursing, or post-acute care settings. Track record of improving census and client retention through building strong relationships. Experience building and scaling small teams (5-10 people). Familiarity with regulatory compliance (HIPAA, state licensing, accreditation) in healthcare. Compensation & Benefits Salary: $90,000-$105,000 annually, based on experience and performance. Annual bonus structure available as well. We're looking for someone results-oriented, so compensation will be reviewed and adjusted based on demonstrated impact in your first year.
