Join a dynamic clinical team dedicated to enhancing patient outcomes through expert pharmaceutical care across all age groups, from newborns to geriatrics. In this role, you will collaborate closely with physicians and other healthcare providers to develop and implement tailored medication plans that align with patient care goals.

Essential Responsibilities

• Participate actively in pharmaceutical care and medication management.
• Complete, verify, and sign required documentation in compliance with legal and policy standards.
• Engage in ongoing training and professional development.
• Review patient medical histories and symptomology to recommend appropriate pharmaceutical therapies.
• Develop customized medication care plans in partnership with healthcare teams.
• Ensure thorough and accurate patient records, including prescription histories, to identify potential drug interactions or contraindications.
• Educate healthcare providers on emerging pharmaceuticals and their proper use.
• Adhere strictly to privacy and security regulations concerning Protected Health Information (PHI).
• Maintain effective communication and professional relationships with patients, team members, and external partners.

Education and Experience

• Doctor of Pharmacy (Pharm.D.) from an ACPE-accredited School of Pharmacy or Bachelor of Science in Pharmacy.
• Board certifications required: BCCCP for Critical Care, BCPS for Infectious Disease, and BCOP for Oncology specialties.
• Current ACLS/ALS certification within 30 days of hire, from recognized organizations such as the American Heart Association or American Red Cross.

Licenses and Certifications

• Valid Pharmacist license in the relevant state.
• Board certification and antimicrobial stewardship training or certification, if applicable.
• American Heart Association Basic Life Support (BLS) preferred.

Preferred Qualifications

• Three to five years of pharmacy experience in a hospital setting.

Knowledge, Skills, and Abilities

• Strong understanding of federal and state pharmacy laws and regulations.
• Proficient computer skills.
• Excellent communication abilities.

Physical Requirements

Medium work involving occasional lifting up to 50 lbs and frequent standing or walking for 26-50% of the workday. Accommodations available as needed.
