Social Services Assistant

Meadowbrook is looking for a compassionate and dedicated Social Services Assistant to work hand in hand with our Social Services Director in supporting the emotional, social, and overall well-being of our residents. Skilled nursing facility experience is preferred , but we are looking for someone who is compassionate, organized, and passionate about advocating for residents and their families. Health, dental, and vision insurance Paid vacation, holiday, and sick time 401(k) with employer match Supportive team environment and culture Responsibilities Assist the Social Services Director with day-to-day social services needs Support residents and families with social, emotional, and psychosocial needs Assist with assessments, care planning, and required documentation Advocate for resident rights, dignity, and quality of life Provide support to residents and families during transitions and adjustments to long-term care Assist with discharge planning and connecting residents with community resources Participate in interdisciplinary team meetings and care planning Help address resident concerns and promote a positive resident experience Maintain compliance with facility policies and state and federal regulations Skilled nursing facility experience preferred.
