Position Details:

• Salary : $40.55-$48.35 based on experience
• Schedule: M-F 8a-5p , occasional evenings and weekends possible

The Registered Nurse (RN) provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant’s condition to primary caregivers, physician and care team.

REQUIRED

1. Must have a valid driver’s license, proof of insurance and have means of transportation.
2. Must have medical clearance for communicable diseases and up-to-date immunizations after having direct participant contact.
3. Current licensure in the state of Kentucky as a Registered Nurse.
4. Current CPR Certification.
5. A minimum of one-year (1) experience in working with the frail or elderly population

1. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.
2. In coordination with the other clinical staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Interdisciplinary Team.
3. Conducts a comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts nursing re-assessment semi-annually and as needed.
4. Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant’s condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.
5. Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.
6. Receives participant complaints, triages, and responds to participant needs.
7. Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.
8. Performs care coordination with outside providers and facilities including hospitals, nursing homes, assisted living facilities, and physician/provider offices. May accompany participants to specialist appointments or make hospital or other facility visits when necessary for coordination of participant care.
9. Provides treatment instructions and nursing education to participants and family.
10. Assists, as directed, in the maintenance of the clinic and exam rooms including stocking of supplies and materials and cleanliness.
11. Maintains accountability for high customer service standards and monitors, evaluates, and recommends opportunities for improved participant and family/caregiver satisfaction.
12. Manages and reviews participant’s records, charts, pertinent information and staff’s documentation. Documents as needed in medical records according to Senior CommUnity Care standards.
13. Maintains the stability and reputation of SCC by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual requirements.
14. Establishes and maintains a productive working relationship with relevant contracted providers.
15. Is responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.
16. Follows and ensures that clinical team follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines.
17. Educates staff in regards to care and treatment for the elderly as needed.
18. Acts within the scope of his or her authority to practice.
19. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
20. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
21. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
22. Performs other duties as required or requested.
