Licensed Practical Nurse (LPN) - Long Term Care

LPN – Post-Acute Care

Schedule: 12H Day shifts, variable scheduling with no guaranteed weekends or days off.

Assignment Details: Contract Length: 5 Weeks. Guaranteed hours are not specified.

Requirements

• Current LPN license required.
• Minimum of 1 year of clinical experience preferred.
• Experience in post-acute skilled nursing rehab is required.
• IV Certification preferred for LPNs.
• Experience with EPIC documentation system required within the past 2 years.
• Dementia capable care certification preferred.

Responsibilities

• Provide nursing care to patients in a post-acute care setting.
• Administer medications and treatments as prescribed.
• Assist with procedures such as phlebotomy and EKGs.
• Coordinate patient care with other healthcare professionals.
• Monitor patient progress and report changes to the care team.
• Educate patients and families about care plans and health management.
