Remote Discharge Care Manager RN Care Coordination

UnitedHealth Group

UnitedHealth Group is seeking a Discharge Care Manager to lead telephonic case management for high-risk members. You will assess needs, coordinate discharge plans, and work with clinical teams to ensure smooth transitions from inpatient care.

The role supports remote work within EST/CST zones, with the potential for collaboration across care teams and documentation of activities to improve health outcomes. An RN license and 5+ years of inpatient experience are required.
