A community hospital in Nyack, NY is looking for a Manager of Case Management to join their team!

Job Function:

• Leads and oversees the hospital's care management function, ensuring patients receive effective, high-quality care while maintaining efficiency and cost awareness.
• Provides leadership and supervision to case management staff, ensuring patients are supported throughout their entire care journey - from admission through discharge and beyond - while promoting continuity of care and positive outcomes.
• Works closely with physicians, care teams, patients and families, and payers to coordinate services and support optimal outcomes.
• Guides decisions related to patient length of stay by applying knowledge of regulatory requirements and payer guidelines, helping to facilitate timely and appropriate discharges.

• BSN required
• Current NYS RN license
• 5+ years of RN experience in acute care setting, including 2+ years experience in case management or utilization review
• 1+ years of leadership experience
• Case Management and/or PRI certification preferred
