Job Description
Title: Registered Nurse - Home Health

Schedule: 8 hours per day

Assignment Details:
- Contract Length: 9 weeks
- Guaranteed Hours: 40 per week

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 2+ years of home health experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

Roles & Responsibilities:
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families regarding care, medications, and home safety.
- Supervise and provide clinical direction to support staff to ensure quality of services provided.
- Monitor assigned cases for compliance with third-party payer requirements.
- Perform on-call responsibilities to support patients and families as necessary.

Compliance / Notes:
- Maintain current licensure certifications and continue education requirements.
- Ability to work flexible schedule and travel locally.
- Maintain confidentiality in all aspects of care.
