Job Summary
Physical Therapist (Outpatient) Sun City Center, FL. This role provides outpatient physical therapy by evaluating assigned patients and developing, implementing, and modifying individualized treatment plans in alignment with patient goals and physician direction. The position also supports documentation requirements, directs discipline-specific therapy services for assigned patients, and contributes to patient education and safe return to community activities. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Monday through Friday, with 8 shift hours and a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ year of outpatient clinical experience; CPR Certification; active/applicable license for the State where work will be performed.
• Preferred Qualifications: Prior successful assignment with the organization; prior successful assignment with the organization previously.
• Additional Information: Completes comprehensive patient evaluations using clinical reasoning to develop treatment plans and goals focused on discharge progression within an established length of stay. Completes medical record documentation, including goals, functional quality indicator assessment, letters of medical necessity, and charge entry in compliance with applicable guidelines and clinic policies and procedures. Ensures care is provided under signed, valid physician orders and within insurance approval when indicated and necessary. Participates in student programming as assigned with appropriate supervision and learning objectives. Adheres to the discipline-specific State Practice Act and other regulatory agencies, maintains professional licensure in good standing, and performs in an ethical manner consistent with the professional code of conduct. Supports performance improvement efforts through feedback and follow-through on action plans.
