LanceSoft is seeking a travel nurse RN Hospice Manager for a travel nursing job in Southington, Connecticut.

RN Hospice Case Manager needed for 8 week contract with option to extend.

Office Location: 81 Meriden Ave, Southington, CT 06489.

Schedule: Mon - Fri 8am-430pm, 40 hrs/wk. No call.

Required: Active/clear CT RN license (or compact)

Associates or Bachelors degree.

Prior hospice nursing experience

• Experience with Homecare Homebase (HCHB)

• Strong case management and interdisciplinary team collaboration skills

• Ability to manage a hospice caseload independently while providing compassionate, patient-centered care.
