Job Overview Primary Care Office Advanced Practice Provider (APRN or PA) provides total patient care in a collaborative model with physicians and the patient care team. Must hold a Florida license in good standing. Responsibilities include patient assessment, care planning, diagnosis, ordering tests, prescribing medication, surgical assistance, home health, DME, and timely documentation. The provider also delivers symptom management, advanced care planning, quality measure compliance, and coordinates care across all providers, families, and caregivers. Palliative and hospice care coordination is performed when appropriate. Essential Duties and Responsibilities Responsible for the total care of all assigned patients in collaboration with the community-based physician and care team. Conduct patient assessments and care planning, develop risk management plans, address quality measures and HCCs, and provide acute interventions, chronic care management, and palliative care services within scope of practice. Provide services to meet patient needs and prevent unnecessary Emergency Department utilization. Document all pertinent medical conditions in the electronic medical record in a timely and CMS-compliant manner. Apply strong clinical skills to improve care for an aging adult population and communicate health information effectively to patients, families, and teams. Demonstrate leadership in nursing process application, initiate interdisciplinary projects, and improve organizational performance. Identify patients who would benefit from skilled nursing/therapy, initiate qualifying processes, and ensure admission to qualifying SNF per CMS guidelines. Instruct and counsel patients on therapeutic regimens and health maintenance. Identify patients needing additional assistance and make referrals to home care, social services, behavioral health, and community resources. Collaborate with Program Manager to oversee assigned Medical Assistant, establish workflow, education/training, and employee improvement plans. Supervisory Responsibilities No direct supervisory responsibilities; may indirectly supervise a Medical Assistant. Qualifications Education and Experience – Master’s degree in nursing; two or more years of related experience or equivalent education/experience. Completion of a master’s degree in nursing preferred. Mathematical Skills – Ability to perform basic arithmetic, calculate rates and percentages, and interpret graphs. Reasoning Ability – Define problems, collect data, establish facts, draw conclusions, and interpret technical instructions. Computer Skills – Computer literate with Microsoft Office (Excel, Word, Spreadsheet) and experience with Athena database software; ability to learn new programs. Certificates, Licenses, Registrations – Current active Florida license as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA). CPR/BLS certifications preferred.
