We are currently seeking a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to join our mobile care team. In this role, you will provide in-home health evaluations, build meaningful relationships with patients, and help bridge gaps in care.

Role Overview The typical visit lasts about 45 minutes, allowing time to connect one-on-one, review medications and medical history, answer questions, and complete a physical evaluation.

What You’ll Do

Provide compassionate, patient-centered in-home health evaluations

Deliver comprehensive assessments, including medication review, medical history, and physical exam

Engage with patients in their homes, assisted living, and senior community settings

Build trusted relationships to support better health outcomes

Travel locally within Los Angeles and Orange County

Maintain accurate clinical documentation

Communicate effectively with the care team as needed

Active NP or PA license in California

Valid driver’s license and reliable transportation
