Overview At FutureCare, Social Workers play a critical role in advocating for residents and improving access to health care services within our facilities. They provide crisis intervention support and promote good health and well‑being for residents. Clinical supervision ensures that FutureCare standards and quality are delivered at the highest level of competency. Benefits Competitive pay: $29.00 – $36.00 per hour Excellent benefit package, including flex/advance pay, paid time off, tuition reimbursement, career growth ladder, employee referral bonus program, employee assistance, and matching 401(k) plan New graduate LBSW and LMSW positions welcomed Responsibilities Coordinate medically related social services to assist residents in maintaining or improving their ability to manage day‑to‑day physical, mental, and psychosocial needs Assist in meeting residents’, families’, and significant others’ psychosocial needs to cope with illness, disability, treatment, or stay in the facility Address advance directives and assist in determining the appropriate medical decision maker Provide individual, family, and group services designed to maximize psychosocial well‑being Promote staff actions that maintain or enhance each resident’s rights and dignity, respecting individuality Assist in developing interdisciplinary care plans for residents Provide support and grief counseling as needed Oversee and coordinate discharge planning to all other settings Qualifications Bachelor’s and/or master’s degree in Social Work Preferred experience in skilled nursing or sub‑acute health care setting Valid Maryland Social Work license Ability to function as a member of an interdisciplinary team Excellent interpersonal skills to relate to residents, families, and community agency personnel Strong prioritization and organization skills to meet deadlines and handle distractions Proficiency in navigating and documenting electronic health record systems Equal opportunity employer.
