Registered Nurse - Case Manager

Role Provides discharge planning for patients by assessing their needs, developing care transition plans, and collaborating with an interdisciplinary team to ensure compliance and effective patient care. Responsibilities Identifies barriers to discharge, assesses high-risk patients, develops discharge plans, and participates in quality improvement initiatives. May also handle utilization management, case management, and act as a liaison with post-acute providers. Qualifications Graduate of an accredited nursing program, with at least three years of acute care experience. Holds valid RN licensure in applicable states, including Illinois, Missouri, Oklahoma, or Wisconsin. Skills Strong assessment, communication, and collaboration skills, with physical ability to perform required tasks. Knowledge of evidence-based practice and adherence to professional nursing standards.
