Part-Time Speech-Language Pathologist (SLP-CCC) – Skilled Nursing & Rehabilitation

Wyomissing Health & Rehabilitation Center is seeking a compassionate and dedicated Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team in Reading, Pennsylvania . This is an outstanding opportunity for a Speech-Language Pathologist who is passionate about providing high-quality, patient-centered care in a Skilled Nursing and Rehabilitation setting.

As part of our interdisciplinary rehabilitation team, you'll have the opportunity to make a meaningful impact by helping residents improve communication, cognition, and swallowing function while enhancing their overall quality of life.

Responsibilities

• Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.

• Develop and implement individualized treatment plans to maximize functional outcomes.

• Provide skilled speech therapy services to short-term rehabilitation and long-term care residents.

• Perform dysphagia assessments and implement appropriate swallowing interventions.

• Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary care team to ensure optimal resident outcomes.

• Educate residents, families, and caregivers regarding treatment plans and communication strategies.

• Complete accurate and timely documentation using Net Health and PointClickCare (PCC) .

• Maintain compliance with all federal, state, and facility regulations.

Qualifications

• Current Pennsylvania Speech-Language Pathologist license.

• Certificate of Clinical Competence (CCC-SLP) required.

• Master's degree in Speech-Language Pathology from an accredited program.

• Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but not required.

• Experience with Net Health and PointClickCare (PCC) is a plus.

• Strong clinical assessment, communication, and organizational skills.

• Passion for providing compassionate, resident-centered care.
