Summary Provides hands on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist. Provides other support services as a member of the care team. Job Duties Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care. Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate. Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate. Minimum Qualifications High School Diploma/GED 1 year Experience in a healthcare setting as a nursing assistant or nursing aide. Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team. Knowledge of medical terminology. Strong time management skills. Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs. Ability to work in patient homes. Ability to work with and support terminally ill patients and their family members/caregivers. American Heart Association Basic Life Support - State of Pennsylvania Upon Hire CNA - Certified Nursing Assistant - State of Pennsylvania Upon Hire Additional Qualifications Home Health/Hospice - must possess and maintain personal car insurance at PA State minimum requirements. Physical Demands Lift over 50 lbs. Push/pull up to 140 lbs. Carry 80 lbs. short distances. Frequent standing/walking, squatting, kneeling, crawling, climbing and reaching overhead.
