The Nurse Practitioner will be responsible for rounding in hospitals and skilled nursing facilities, reviewing patients currently on our services and updating care plans accordingly. Under the direction of the Medical Director, the Nurse Practitioner will assess, review medication, create an appropriate care plan, provide and evaluate care. In addition, the Nurse Practitioner will assist with advanced directives and discuss goals of care and the disease process, progression, and grief process with patients and caregivers. The Nurse Practitioner will work with the Director of Patient Care Services and Executive Director to provide education on Palliative and Hospice care within the community as well as agency staff regularly. Requirements Current license from the state Board of Registered Nursing Master’s Degree from an accredited Nurse Practitioner’s Program National Certification as a Nurse Practitioner or certification eligible.
