Job Description:
RN Dialysis NavigatorThe RN Dialysis Navigator serves as the clinical resource and primary coordinator for patients requiring dialysis services throughout Bay Area Hospital. This position facilitates safe, timely, and coordinated dialysis care across the continuum by collaborating with patients, families, nephrologists, nursing staff, providers, contracted dialysis services, and other members of the interdisciplinary care team. The RN Dialysis Navigator provides clinical expertise related to dialysis care, supports transitions and continuity of care, identifies and assists in resolving barriers to treatment, and promotes adherence to established standards, policies, and evidence-based practices. The position also supports staff education, quality improvement, program development, and ongoing evaluation of dialysis services.EXPECTATION FOR ALL EMPLOYEES

PRINCIPLE DUTIES AND RESPONSIBILITIES
Coordinates and provides safe, individualized dialysis care, including assessment, planning, implementation, monitoring, and evaluation of patient treatment.
Independently sets up, operates, monitors, disinfects, and troubleshoots the Tablo Hemodialysis System and performs prescribed dialysis therapies.
Manages dialysis access, including central venous catheters, arteriovenous fistulas, and grafts, in accordance with established standards and procedures.
Monitors patient response, laboratory results, treatment parameters, and clinical status; recognizes complications and initiates appropriate interventions.
Administers medications, blood products, and prescribed therapies in accordance with provider orders and organizational policies.
Coordinates dialysis services and patient care with nephrologists, providers, bedside nursing, pharmacy, case management, and other members of the interdisciplinary team.
Provides patient and family education regarding dialysis treatment, vascular access, fluid management, and ongoing care needs.
Serves as a clinical resource for dialysis-related care and supports staff education, orientation, and competency development.
Performs and documents required water quality testing, equipment safety checks, treatment documentation, and other regulatory requirements.
Supports continuity of care and transitions between inpatient services, outpatient dialysis providers, and other care settings.
Participates in quality assurance, process improvement, chart audits, regulatory readiness, and monitoring of dialysis program outcomes.
Assists with the development and implementation of dialysis policies, procedures, standards of care, and evidence-based practices.
Supports dialysis equipment and supply readiness and coordinates equipment concerns with leadership, Biomedical Engineering, and appropriate resources.
Participates in the departments on-call rotation and responds to emergent dialysis needs in accordance with department requirements.
Participates in the departments on-call rotation, including nights, weekends, and holidays, and responds to emergent dialysis needs based on patient care and departmental requirements.
Performs other duties as assigned.
SKILLS AND ABILITIES
Demonstrates strong clinical knowledge of dialysis nursing, including hemodialysis, vascular access, and care of patients with kidney disease.
Uses sound clinical judgment, critical thinking, and problem-solving skills to identify and address patient care concerns.
Effectively coordinates care and communicates with patients, families, providers, staff, and external dialysis partners.
Demonstrates strong organizational skills with the ability to prioritize multiple clinical and operational responsibilities.
Demonstrates leadership and knowledge of staff education, quality improvement, patient safety, infection prevention, and applicable regulatory standards.
EDUCATION/CERTIFICATIONS/LICENSES/DEGREES
Graduate of an accredited school of professional nursing, required.
Current unencumbered Oregon Registered Nurse (RN) license, required.
American Heart Association Basic Life Support (BLS) certification, required.
Bachelor of Science in Nursing (BSN), preferred.
EXPERIENCE
Minimum of three (3) years of recent dialysis experience, required.
Demonstrated experience caring for patients receiving hemodialysis and managing dialysis-related clinical needs.
