Responsibilities

• Assess patient needs in either inpatient or outpatient environments and develop personalized treatment plans
• Implement physical therapy interventions aimed at improving mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and their families on therapeutic goals, discharge planning, and recommended home exercises
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in line with facility and regulatory requirements
• Engage in care team rounds and interdisciplinary meetings to ensure a holistic approach to patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications such as neurological or geriatric are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
