Job Title Physical Therapist Location Kentucky | Outpatient / Inpatient Rehab Schedule / Job Type Contract | Days | 4-Week Assignment Key Responsibilities Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals aimed at progressing patients toward discharge within an established length of stay Collaborate with an interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in all aspects of caseload management Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with applicable guidelines Participate in the student program as assigned, providing a positive learning environment with appropriate supervision, learning objectives, and student evaluations Adhere to all safety policies and procedures, including safe patient handling, materials handling, infection control, and hazard reporting, while maintaining active professional licensure in good standing Required Qualifications Active Physical Therapist license in good standing in the state of Kentucky Compliance with state-mandated continuing education requirements Adherence to the discipline-specific State Practice Act and applicable regulatory agency standards Ability to perform in an ethical manner in accordance with the discipline-specific professional code of conduct Preferred Experience / Skills Experience in an outpatient neurological setting Clinical rotation experience in outpatient therapy if direct outpatient neuro experience is not available Familiarity with performance improvement methodologies and organizational quality initiatives Why Join Us Work in a setting that prioritizes both patient and employee safety at every level of care Opportunity to contribute to a collaborative, interdisciplinary team environment while supporting your own professional growth and development Concentric Healthcare Staffing Job ID 793418.
