Job Summary:

Responsibilities:

• 
  Administer medications and treatments accurately and timely.
  
  
• 
  Monitor residents health status and report changes to the healthcare team.
  
  
• 
  Follow protocols for infection control and safety procedures.
  
  
• 
  Document patient care activities thoroughly and promptly.
  
  
• 
  Assist with activities of daily living and provide emotional support.
  
  
• 
  Comply with documentation and regulatory requirements, including wearing identification badges.
  
  

Qualifications:

• 
  Hold an active LVN or LPN license in the state.
  
  
• 
  Bring a valid state or federal-issued ID and wear a photo badge during shifts.
  
  
• 
  Complete or be willing to complete the COVID-19 vaccination, or present a valid religious or medical exemption.
  
  
• 
  Demonstrate excellent communication and interpersonal skills.
  
  
• 
  Have prior experience in long-term care or skilled nursing facilities preferred.
  
  
• 
  Maintain reliable attendance and professionalism.
