Hospital Medicine Advanced Practice Provider (Nights) (Multiple Openings)

Organization DGIM - Hospital Medicine Job Summary The Advanced Practice Provider (APP) works in the acute care hospital setting as a hospitalist, managing care of acute/complex chronically ill patients. The APP is an integral member of a multidisciplinary health care team, performing assessments, diagnostics, and ongoing patient management from step‑down to discharge. The APP may also provide continuity during follow‑up outpatient visits. The role requires evidence‑based practice, clinical competence, sound judgment, and professional communication. Qualifications Licensed Nurse Practitioner (NP) OR Physician Assistant (PA) Key Responsibilities Assess health status, perform complete history and physical examinations for acute and complex patients. Order and collect diagnostics, interpret results, and perform procedures when indicated. Serve as first responder for patient status changes and participate in procedural/resuscitative interventions. Assess risks of medication side effects, immobility, nutrition, fluid/electrolyte balance, immunocompetence, invasive interventions, and diagnostic procedures. Coordinate daily rounds with attending teams, present 24‑hour patient reports, collaborate in care plans, and follow assigned patients. Collaborate with interdisciplinary team to diagnose acute and complex chronic conditions, formulate differential diagnoses, and determine appropriate interventions. Develop and modify individualized care plans based on evidence‑based guidelines. Prescribe diagnostic strategies and therapeutic interventions, including pharmacologic, non‑pharmacologic, and surgical options. Perform advanced procedures such as minor suturing, complex wound management, line and tube placements and removals as privileges allow. Maintain ongoing communication with team, patients, families, and staff to promote continuity of care. Document clinical details via daily progress notes, transfer and discharge summaries, clinic notes, etc., ensuring compliance and capturing patient acuity. Demonstrate professional practice behaviors: pre‑instruction, mentored education, participation in Grand Rounds, faculty meetings, M&M presentations, continuing education, and professional memberships. Certifications Licensed Nurse Practitioner – Tennessee Physician Assistant Certified – Tennessee Registered Nurse – Tennessee Experience Less than 1 year Relevant work experience in acute or complex patient care. Education Master’s degree in Nursing or related field.
