Care Manager

Pride Health is seeking a Care Management Associate for a full-time hybrid opportunity in New York, NY. Pay Rate: $23/hour to $28/hr Responsibilities Handle inbound and outbound member calls Schedule appointments for Care Management staff Maintain member files and track membership status Assist members with scheduling medical appointments and services Coordinate communication with home care vendors regarding member services Enter authorizations and service approvals into the authorization system Monitor authorization queues to ensure timely processing Track member hospital admissions, ER visits, nursing home stays, and other care-related events Coordinate required documentation for service authorizations and re-authorizations Review documentation for accuracy and completeness Support care coordination activities and administrative workflows Qualifications High School Diploma or GED required Minimum 1 year of related work experience Customer service or call center experience preferred Understanding of medical terminology preferred Knowledge of ICD-9 and CPT-4 codes preferred Prior managed care experience preferred Strong communication, organizational, and documentation skills If you are interested, please send your updated resume to View email address on click.appcast.io or message me directly.
