Care Navigator: Patient Support & Access Advocate
The Institute for Family Health
The Institute for Family Health seeks a Care Navigator to join an interdisciplinary team addressing patient needs that can affect treatment adherence. You will accompany patients to appointments, promote healthy living, provide social support and distribute health education literature. You will conduct home visits and community outreach, ensure engagement, and obtain health home consents while following evidence-based guidelines and performing pre-visit planning and case management. #J-18808-Ljbffr
