We are seeking a compassionate and skilled Evening Shift RN Nurse to join our team at Mount Airy Nursing and Rehab, one of the nation's largest providers of post-acute care. Our facilities offer a range of services, including skilled nursing rehabilitation centers, long-term care centers, assisted living communities, independent rehabilitation centers, and long-term acute care hospitals (LTACH).

About the Role:

As an Evening Shift RN Nurse, you will provide high-quality nursing care to our patients while working evening shifts. You will assess patient needs, develop individualized care plans, and implement evidence-based interventions. You will collaborate with our interdisciplinary team to ensure seamless transitions and optimal patient outcomes.

Responsibilities:

• Provide direct patient care in a fast-paced, dynamic environment

• Conduct initial and ongoing patient assessments, identifying physical, emotional, and social needs

• Develop and implement individualized care plans, incorporating current research and evidence-based practices

• Collaborate with the interdisciplinary team to ensure comprehensive care and optimal patient outcomes

• Participate in quality improvement initiatives to enhance patient care and satisfaction

• Maintain accurate and up-to-date patient records, meeting regulatory requirements and facility standards
