We are seeking a dedicated Registered Nurse with proven home health experience, including OASIS documentation proficiency, for a full-time, 13-week contract assignment. This role is ideal for RNs confident in clinical skills, detail-oriented with documentation, and comfortable managing a structured schedule making a real difference in patients lives in the comfort of their own homes.

Schedule:

• Full-time, Monday through Friday
• On-call duties may be assigned
• Daily visit expectations

Qualifications:

• Active RN license
• At least one year of recent home health RN experience
• Required OASIS assessment and documentation experience
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and valid drivers license
• Strong time management and patient communication skills

Responsibilities:

• Deliver skilled nursing care in patients homes, including assessments, interventions, and education
• Complete accurate OASIS documentation in a timely manner
• Coordinate care and updates with physicians and the broader multidisciplinary team
• Manage wound care, administer medications, monitor disease processes, and educate patients and families
• Report changes in patient conditions promptly to the clinical team
