The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.
Under general direction, the Clinical Pharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include:
The essential duties of this position include but are not limited to:
Interviews and examines patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed.
Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.
Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc.
Participates in the development and revision of medication use policies and guidelines.
Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records.
The 2454 Clinical Pharmacist also performs other related duties as assigned.
Nature of positions: Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. Some positions may require employees to work directly with individuals within locked jails and psychiatric facilities, the homeless population and/or individuals who have or are carriers of infectious/communicable diseases.
Minimum Qualifications
License: Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy. (Please attach a copy of license);
- License status must be "Valid and Current" according to search.dca.ca.gov/
AND
Experience: One (1) of the following:
A. Completion of six (6) months of a pharmacy residency program; OR
B. Completion of six (6) months of a post-graduate pharmacy fellowship program; OR
C. Two (2) years of professional level experience as a Clinical Pharmacist (equivalent to class 2454 Clinical Pharmacist within the City and County of San Francisco); OR
D. Possession of a Board Certification in a pharmacy specialty area.
One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

CONDITION OF EMPLOYMENT: Appointees to positions subject to oversight by any Organized Medical Staff within the San Francisco Department of Public Health (or other applicable facilities) must maintain all required credentialed statuses, including membership, affiliated professional status, valid and current licensure, hospital privileges, and/or standardized procedures, and remain in good standing with the respective facility’s Medical Staff in accordance with bylaws, policies, rules, and regulations.
Possible Special Conditions (some positions may require the following):
Completion of a PGY2 Pharmacy Residency program in emergency medicine, critical care, or toxicology; OR two years of full-time professional level experience as a clinical pharmacist in an Emergency Department (ED) setting.
Completion of a PGY2 Pharmacy Residency program in psychiatry; OR two years of full-time professional level experience as a clinical pharmacist in a psychiatric setting.
Bilingual fluency in a variety of languages depending on the department’s needs. Only those eligibles that pass the bilingual proficiency exam will be considered for bilingual positions.
The ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department.
Recent and verifiable experience: (1) integrating, supporting and maintaining an Electronic Health Record (EHR) or Pharmacy Information System (PIS); and (2) implementing, developing, and evaluating Computerized Prescriber Order Entry (CPOE), within the last three years.
DESIRED QUALIFICATIONS: The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.
Acute Care practice:
Demonstrated experience with the acute care medicine patient population.
Clinical and/ or lived experience with diverse communities.
Excellent oral and written communication and interpersonal skills.
Ambulatory Care practice:
Completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship.
Fluency in Spanish and/or Cantonese is highly desired.
Mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities.
Meets requirements for prescribing as required by Standardize Procedure for Ambulatory Care Pharmacist within San Francisco Health Network.
Behavioral Health practice:
Completion of a psychiatric pharmacy residency.
Clinical experience working in substance use disorder or mental health settings.
Fluency in Spanish and/or Cantonese is highly desired.

Board Certification as a Psychiatric Pharmacist.
Possession of a valid Advanced Practice Pharmacist license in California.
Long Term Care practice:
Completion of a geriatric pharmacy residency.
Certification in Geriatric Pharmacy.

Training and Experience Evaluation (Weight: 100%): Candidates will be email a Training and Experience Evaluation Supplemental Questionnaire at any point in the recruitment process. The Supplemental Questionnaire is designed to measure knowledge, skills and/or abilities in job related areas that have been identified as critical for satisfactory performance in this position. All relevant experience, education and/or training must be on the application in order to be reviewed in the scoring process. Candidate responses on the Supplemental Questionnaire cannot be changed once submitted.
Candidates must achieve a passing score on the Training and Experience Evaluation exam in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score.
Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.
Language Proficiency Exam: For positions requiring bilingual fluency, candidates deemed qualified who meet the special language condition must pass a departmentally approved language proficiency exam prior to appointment. Only those eligible candidates who pass the bilingual proficiency test will be considered for the bilingual position. Applicants must indicate on the application form the language(s) in which they claim proficiency.
Certification: The certification rule for the eligible list resulting from this examination will be the Rule of the List.
Eligible List/Score Report: A confidential eligible list of applicant names that have passed the civil service examination process will be created and used for certification purposes only. An examination score report will be established, so applicants can view the ranks, final scores and number of eligible candidates. Applicant information, including names of applicants on the eligible list, shall not be made public unless required by law. However, an eligible list shall be made available for public inspection, upon request, once the eligible list is exhausted or expired and referrals resolved. The eligible list/score report resulting from this civil service examination process is subject to change after adoption (e.g., as a result of appeals), as directed by the Human Resources Director or the Civil Service Commission.
