Hospice Home Health Registered Nurse (RN)We are seeking a compassionate and experienced Hospice Home Health Registered Nurse (RN) to join our team serving the Greater Lake Charles area. This position is responsible for providing exceptional end-of-life care, coordinating patient services, and managing a caseload of hospice patients in the home setting. The ideal candidate is patient-centered, adaptable, and committed to delivering high-quality care while supporting patients and their families through every stage of the hospice journey.Schedule & Work DetailsLocation: Greater Lake Charles areaSchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Hour Guarantee: 40 hoursOn-Call Requirement: NoneWeekend Requirement: NonePay Rate: $51.00 - $53.00 per hourMileage Reimbursement: Yes, reimbursed between patient visits (from first patient to last patient)Key ResponsibilitiesManage and coordinate care for an assigned caseload of hospice patients.Conduct comprehensive patient assessments and ongoing evaluations.Develop, implement, and update individualized care plans based on patient needs.Provide skilled nursing care and symptom management in accordance with hospice standards and physician orders.Educate patients, families, and caregivers regarding disease progression, comfort measures, medications, and available resources.Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and therapists, to ensure coordinated care.Maintain accurate, timely, and compliant clinical documentation.Advocate for patient comfort, dignity, and quality of life.Participate in care conferences and communication with family members and healthcare providers regarding patient status and goals of care.Required QualificationsActive Registered Nurse (RN) license.Previous hospice nursing experience required.Strong clinical assessment and case management skills.Ability to work independently in a home-based setting.Excellent communication, organizational, and time management skills.Valid driver's license and reliable transportation.Preferred QualificationsExperience with Homecare Homebase (HCHB).Previous home health experience.Strong patient-centered care philosophy.Positive, flexible, and team-oriented attitude.Ability to build meaningful relationships with patients, families, and interdisciplinary care teams.Experience managing complex patient needs and end-of-life care planning.Pride Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, pet insurance and employee discounts with preferred vendors.
