Mental Health Case Manager – Jail Services

Major duties and core competencies include:
- Conduct comprehensive assessments, including whole-person and Health-Related Social Needs (HRSN) assessments, to identify participant strengths, barriers, risks, and service needs.
- Develop, implement, and monitor individualized Re-Entry Care Plans that are strength-based, participant-centered, culturally responsive, and aligned with identified goals and service needs.
- Provide targeted re-entry case management services in accordance with Washington State's Medicaid Re-Entry Demonstration Initiative (1115 Waiver), agency standards, and clinical recommendations.
- Coordinate access to behavioral health treatment, substance use disorder services, medical care, housing resources, transportation, employment supports, benefits, and other community services.
- Serve as an active member of the multidisciplinary treatment team by contributing clinical observations, recommendations, and participant progress updates.
- Maintain timely, accurate, and complete documentation of all services, contacts, assessments, and care coordination activities in accordance with federal, state, and agency regulations.
- Develop and maintain collaborative relationships with community agencies, service providers, and referral partners to improve participant access to resources and support services.
- Demonstrate professionalism, ethical decision-making, cultural humility, and adherence to agency values and professional standards.
Required qualifications include:
- Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Counseling, or a closely related field preferred.
- Washington State Department of Health credential required or ability to obtain and maintain required credentialing.
- Minimum of one year of paid or volunteer experience working with individuals experiencing mental health, substance use, co-occurring disorders, or related behavioral health conditions.
- Working knowledge of behavioral health recovery principles, trauma-informed care, and participant-centered service delivery.
- Familiarity with applicable Washington State behavioral health regulations, including RCW 71.05 and WAC 246-341.

This position serves participants with diverse backgrounds and complex behavioral health, substance use, medical, housing, legal, and social service needs. Success in this role requires strong interpersonal skills, sound judgment, adaptability, independent decision-making, and the ability to coordinate services across multiple systems while maintaining participant engagement and regulatory compliance.
