This position is ideal for an LPN who enjoys working independently while advocating for patients and helping them achieve the best possible outcomes within the comfort of their home. As a Home Health Licensed Practical Nurse – LPN, you will provide home-based care for patients recovering from illness, injury, surgery, or chronic conditions while working closely with RNs, physicians, and interdisciplinary care team members. Responsibilities of Home Health Licensed Practical Nurse – LPN:

• Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions under the supervision of an RN
• Monitor patient condition, obtain vital signs, and perform routine nursing assessments
• Document patient progress and report changes in condition to RN supervisor
• Assist with implementing individualized patient care plans
• Educate patients and families on medications, treatment plans, self-care, and home safety
• Collaborate with physicians, RNs, CNAs, aides, and interdisciplinary team members to coordinate patient care
• Assist with discharge planning and ensure continuity of care
• Maintain accurate and timely documentation

Qualifications of Home Health Licensed Practical Nurse – LPN:

• Graduate of an accredited practical nursing program
• Active LPN license in the state of Virginia
• Minimum of 1 year of LPN experience (home health preferred) or completion of office preceptor program
• CPR certification and TB skin test required
• Knowledge of state and federal home health regulations
• Strong clinical judgment, communication, and critical thinking skills
• Compassionate, dependable, and patient-focused
• Ability to work independently in a home setting
