Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and caregivers on therapeutic exercises and home programs to support continued recovery

Required Qualifications

• Active Physical Therapist (PT) license in the state of Colorado
• Graduate of an accredited Physical Therapy program
• Prior experience working in a clinical or rehabilitative setting
• Strong documentation and communication skills

Preferred Experience / Skills

• Previous experience in a skilled nursing facility (SNF) environment
• Familiarity with Medicare Part A and Part B documentation requirements
• Experience with interdisciplinary team collaboration in a post-acute care setting
