Case Manager Trainer

We are currently seeking a full-time Case Manager Trainer to support the ongoing demand for case management services in Hennepin County. In this role, you will lead the onboarding and training of new waiver case managers, serving as a primary point of contact during their first 90 days while providing ongoing mentorship, guidance, and performance feedback. You will collaborate closely with the Case Management Services Director and supervisors to ensure training effectiveness and high-quality service delivery. Job Summary:

• Lead and manage the onboarding and training process for new Case Managers, serving as their primary point of contact during the first 90 days.
• Provide ongoing mentorship, coaching, and case consultation to support staff development and performance.
• Collaborate with leadership to evaluate training progress, update materials, and ensure compliance with program and licensing requirements.
• Maintain a caseload and deliver direct case management services, including providing coverage when needed.
• Support high-quality, person-centered service delivery by guiding staff, monitoring performance expectations, and promoting best practices across the team.

Required Qualifications:

• Bachelor's degree in social services, psychology, human services, or a related field
• Minimum of one (1) year experience providing case management services to individuals with developmental disabilities or related conditions
• Valid driver's license with up-to-date auto insurance and an acceptable motor vehicle record
• Must be able to pass DHS Background Check

Preferred Qualifications:

• License in Social Work preferred, but not required.

Job Type: Full-time

Pay: $27.00 - $30.00 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
• Monday to Friday

Work Location: Hybrid. Requires reporting to our Edina office up to two days per week when new hires are training. Otherwise fully remote.
