We are a beautiful, small, 42 bed assisted living located in the heart of Niskayuna, convenient to Schenectady, Latham, and surrounding areas. We'd love to review your application and host an interview with our dynamic team.Position Summary: LPNs (Licensed Practical Nurses) are responsible for medications, treatments, and staff oversight under the direction of the RN. In this position you will bring trust, comfort, and empathy to all resident interactions and make a positive difference in the lives of our residents.Job Specific CompetenciesPrincipal Responsibilities:Core Competencies:Supports the Trinity/SPHP Vision, Mission, Values in all encounters with residents and familiesAdheres to the SPHP Code of ConductDemonstrates accountability as an individual and team member in meeting basic resident and family needsClinical Competencies:LPNs provide direct and indirect care under the direction of an RN including procedures, treatments, and related duties as assigned and within the NYS Scope of Practice for an LPN.Resident support – Deliver compassionate, person-centered care while fostering a sense of dignity and independence for each resident.Medication administration – Conduct daily medication pass with accuracy and care in accordance with physician orders and facility guidelines.Shift leadership – provide leadership for the care team during your shift, including guidance and support.Team collaboration – work closely with resident assistants, other nurses, and management in a cooperative, respectful environment.Consult with the RN on any changes in a care plan or schedule and communicate with patients and their families about their care plan.Observe, measure, record, and report clinical data relating to each resident's health status within licensure requirements.Attends and actively participates in in-servicing, orienting and educational activities.Education Requirements:Must be a high school graduate.Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).Must be licensed with current registration in New York.Must be able to:Stand, walk, and lift for long periods of timeRead and write using medical terminologyWorking knowledge of computers including electronic medication administration records, processing of orders, checking email, and preforming required in-servicing, along with use of other required applicationsHave the visual and auditory acuity adequate to perform observations and patient careMaintain composure and positivity in a high stress environmentPromote physical, cognitive, and psychosocial well-being of each residentWork with residents in a highly personal manner during stages of acute and chronic illnessPay Range:$24.00 - $31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
