Responsibilities
- Provide direct and indirect skilled nursing care for residents, under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
- Contribute to the development of an age-appropriate, culturally relevant plan of care, involving the resident, family, and healthcare team to achieve outcomes.
- Recommend revisions to the plan of care based on resident outcomes and coordinate referrals as needed.
- Collaborate with residents/families and interdisciplinary team to ensure quality care and a safe environment.
- Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; demonstrate leadership under supervision.

Requirements
- NY LPN licensure (active)
- BLS certification (AHA or RQI)
- Minimum 1 year experience as an LPN
- Experience serving in an LPN capacity at a long-term care facility
- Ability to provide care to adult and geriatric residents
- Must be able to participate in holiday season coverage and start full-shift orientation on a Tuesday
