RN Care Coordinator, ECM & Care Team Leader

MLK Community Healthcare seeks an RN Coordinator for the Enhanced Care Management program. You will coordinate care for enrolled patients, participate in Interdisciplinary Care Team meetings, and develop patient-centered care plans with emphasis on adherence and follow-up with PCP and behavioral health providers.
