Case Manager

The Case Manager provides exceptional patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program. This role acts as a liaison between the patient, family, nursing staff, physicians, multidisciplinary teams, and patients' healthcare benefactors to optimize outcomes.

Key Responsibilities

• Serve as a consultant to the healthcare team on specific patient needs.
• Coordinate the continuum of care for a defined population of patients from pre-admission through post-discharge.
• Ensure that patients and families navigate efficiently through the course of hospitalization.

Patient Care Focus

This role involves working with a diverse patient population diagnosed with acute psychiatric, behavioral, or mental health disorders, providing evaluation and treatment tailored to the needs of patients of all ages.

Qualifications

• Strong communication and interpersonal skills to effectively collaborate with various stakeholders.
• Demonstrated ability to manage complex care processes and optimize clinical outcomes.
• Experience in case management or related healthcare field is preferred.
