Key Responsibilities\n

In this role, you will:

  • Conduct comprehensive psychosocial assessments of patients to identify their needs, strengths, and mental health challenges.

  • Deliver exceptional counseling services, focusing on disease management and health education, to empower patients in their mental health journey.

  • Collaborate with patients and an interdisciplinary healthcare team to develop and implement personalized care plans that address behavior modification, coping strategies, and lifestyle adjustments.

  • Facilitate goal-setting sessions—synchronous and asynchronous—with individual clients aimed at managing chronic conditions alongside associated mental health challenges.

  • Engage regularly with patients through tailored outreach efforts to elicit behavior change, while responding to their unique preferences and needs.

  • Thoroughly document patient interactions, progress, and care plans in our electronic health record (EHR) system, ensuring compliance with regulatory standards and facilitating continuity of care.

Qualifications\n

  • A Master's degree in Social Work from an accredited institution is required.

  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
