Cardiac Rehab Nurse – Overview The position coordinates the care of Phase I Cardiac Rehab patients at the hospital. It is a casual position; hours per week are not guaranteed and will vary based on staffing needs. Hours are Monday, Wednesday, Friday 6:30 am‑6 pm, Tuesday and Thursday 7:30 am‑3 pm. The role covers Delnor and CDH (Wheaton) Cardiac/Pulmonary Rehab locations. Key Responsibilities Provide patient initial assessment & orientation. Develop rehabilitation plan addressing risk stratification, risk factors & exercise/functional status. Develop patient rehabilitation discharge goals promoting transition back to community health & exercise resources. Communicate rehabilitation plan & discharge to ordering physician(s). Provide & document patient care per department policy & external regulatory requirements. Required Qualifications Bachelor of Science in Nursing. BLS. ACLS (within 6 months of hire). Current state of IL RN License in good standing. Basic computer knowledge. Ability to communicate effectively in English, both verbally and in writing. Excellent customer service skills. Preferred Qualifications One year of experience in pertinent clinical specialty.
