Penn Medicine is seeking a full-time, day shift Ambulatory Clinical Pharmacy Specialist for downtown Philadelphia.
The Clinical Pharmacy Specialist Ambulatory Care develops, implements, and coordinates advanced clinical pharmacy services across ambulatory practice settings. Working collaboratively within interdisciplinary care teams, the specialist provides comprehensive medication management for patients with acute and chronic conditions, with a focus on optimizing therapeutic outcomes, minimizing medication-related risk, and supporting evidence-based prescribing. Responsibilities include conducting detailed medication assessments; supporting individualized medication and disease-state management through collaborative care models; providing longitudinal follow-up for patients with complex or high-risk medication needs; and delivering targeted medication education to patients and providers. The role also contributes to clinic-level quality and safety initiatives and promotes the safe, effective, and cost-conscious use of medications aligned with clinical and operational priorities.
Minimum Requirements:
- Bachelor's of Science in Pharmacy from an accredited College of Pharmacy or a PharmD from accredited college of pharmacy (required)
- Completion of an ASHP- accredited PGY1 Pharmacy Residency (required)
- Completion of an ASHP- accredited PGY2 Pharmacy Residency (preferred)
- Licensed Pharmacist in the state of PA and NJ (required)
