Primary Care Physician

Position Overview:

We are seeking a Primary Care Physician dedicated to delivering high-quality, patient-centered care to Medicare populations within a modern network of community-based clinics. This role focuses on a value-driven healthcare approach, emphasizing preventive care, improved patient outcomes, and reduced hospital utilization rather than service volume. Physicians are supported by robust care teams, allowing them to concentrate on meaningful patient interactions and clinical excellence.

This opportunity is ideal for providers passionate about caring for older adults. Recent graduates are welcome to apply. Exceptional candidates may also be evaluated for a leadership role as a Center Medical Director.

Key Responsibilities:

• Evaluate, diagnose, and treat patients in an outpatient clinic setting
• Provide comprehensive primary care services
• Prescribe medications and manage treatment plans
• Maintain accurate and timely patient records using EMR systems (e.g., Canopy, Greenway)
• Partner with leadership teams to achieve clinical quality benchmarks
• Collaborate with Practice Managers to guide and support care team operations
• Perform additional duties as needed

Required Qualifications:

• MD or DO degree from an accredited institution
• Board certified or board eligible in Internal Medicine or Family Medicine
• Current, unrestricted state medical license in good standing
• Active DEA registration
• Authorization to work in the United States

Preferred Qualifications:

• Fellowship training in Geriatrics
• Prior experience treating geriatric patients
• Familiarity with value-based care models
• Experience working in team-based, collaborative healthcare environments
• Background in outpatient primary care
• Bilingual skills, where applicable
