We are seeking a compassionate and clinically drivenPhysical Therapist (PT)to join our rehabilitation team. In this role, you'll provide skilled physical therapy services, develop individualized treatment plans, and work closely with residents to help them achieve meaningful functional goals.
West Roxbury Health & Rehabilitation Center is a 76-bed skilled nursing and rehabilitation community in West Roxbury, Massachusetts, providingShort-Term Rehabilitation and Long-Term Care, along with specialized support includinghospice care and respite care. Our interdisciplinary team provides personalized nursing care, rehabilitation services, and clinical support while creating a welcoming environment centered on resident dignity, comfort, and quality of life. As a valued member of our interdisciplinary team, you'll collaborate with Physical Therapist Assistants, nursing staff, providers, and other healthcare professionals to support resident progress, promote safe mobility, and improve quality of life through personalized care.

Responsibilities
What You'll Do
As aPhysical Therapist, you will evaluate residents, develop individualized plans of care, and provide skilled physical therapy interventions designed to improve mobility, strength, balance, safety, and functional independence.
Physical Therapist Responsibilities:
Perform comprehensive physical therapy evaluations and assessments based on resident needs
Develop individualized treatment plans and measurable therapy goals in accordance with physician orders
Provide skilled physical therapy treatments within the scope of practice and established plan of care
Treat residents with a variety of orthopedic, neurological, and functional mobility needs
Help residents improve strength, balance, coordination, endurance, mobility, gait, and functional abilities
Implement therapeutic exercises, functional activities, gait training, balance interventions, and other appropriate treatment approaches
Monitor resident response to treatment and modify interventions as clinically appropriate
Evaluate resident progress and make recommendations regarding continued therapy, discharge, or maintenance needs
Provide education to residents, families, and caregivers regarding exercises, mobility strategies, safety, and discharge recommendations
Collaborate with Physical Therapist Assistants regarding treatment plans, resident progress, and clinical recommendations
Participate in resident care planning and interdisciplinary team meetings
Participate in discharge planning and provide recommendations to support successful transitions
Maintain accurate and timely clinical documentation in accordance with state and federal regulations
Communicate resident progress, changes in condition, and treatment recommendations to appropriate members of the care team
Provide clinical direction and support to Physical Therapist Assistants as appropriate
Assist with maintaining a safe, clean, and organized therapy environment
Participate in continuing education, in-service training, and departmental meetings
Maintain resident confidentiality, privacy, and dignity in all interactions
Follow all safety, infection control, and regulatory standards
Perform other duties as assigned
Our Physical Therapists have the opportunity to work with residents experiencing a variety of rehabilitation needs, including:

Qualifications
What We're Looking For
Physical Therapist Qualifications:
Graduation from an accredited Physical Therapy program required
Current Massachusetts license to practice as a Physical Therapist required
Current CPR/BLS certification required
Long-term care, skilled nursing, or rehabilitation experience preferred
Experience working with geriatric and subacute rehabilitation populations preferred
Strong clinical reasoning, evaluation, and treatment planning skills
Ability to develop and implement individualized plans of care
Excellent communication and interpersonal skills
Ability to collaborate effectively with residents, families, and interdisciplinary healthcare teams
Ability to provide clinical direction and support to Physical Therapist Assistants as appropriate
Compassionate, dependable, and resident-focused approach to care
Passion for helping residents achieve meaningful functional goals
Ability to meet applicable state and federal regulatory requirements
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
